Mrs C – Crumlin
Health Caseworker visited Mrs C as part of the Hospital to Home Pilot Project. There was no central heating at the property the only form of heating consisted of a gas fire to the ground floor living room and a portable electric heater to the bathroom and main bedroom. Client suffered Pneumonia which turned to Pleurisy last winter. Caseworker arranged for new electric wiring and fuse box to be updated. A NEST grant was applied for and a new central heating system and radiators have been installed throughout the property. Client is now confident that she will be safe, warm and comfortable in her home this winter.
Mrs B – Rhymney
Health Caseworker visited Mrs B as part of the Hospital to Home Pilot Project. Caseworker carried out a Healthy Homes check and discovered that the heating system at the property was old and had broken down in the past. There was a partial grant available from clients’ energy supplier but client was struggling to pay her share of the cost which was £1150. The Health Caseworker applied to CRC for a HealthThrough Warmth grant for £750 so that work could commence to replace the old back boiler. A referral was also made to Social Services to help client with bathing as she was struggling to get in and out of the bath. Client has been awarded a bath lift and now feels much safer, warmer and independent in her home.
